Home Health Nurse Job Description

A home health nurse is a licensed healthcare professional that provides health care services in a patient’s private home.

Home health nurse services are typically provided on a periodic basis, such as weekly, every other week, or monthly.

Patients are referred to home health nurses by a physician, a home health agency, or a hospital.

Home health nurses help patients with a variety of needs such as wound care, bathing, mobility, medication management, and feeding.

Home health nurses may work as independent contractors or as employees of home health agencies.

Job Brief:

We’re looking for a Home Health Nurse to work with us in providing care to patients in their homes. The Home Health Nurse will be responsible for assessing the patient’s needs, developing a care plan, and providing nursing care to the patient.

Home Health Nurse Duties:

• Actively participate in assessing and evaluating patient care needs, developing and administering nursing plans of care, and executing treatment plans
• Develop, implement, and evaluate nursing plans of care
• Perform nursing assessments, including health history, physical assessment, and mental status
• Implements nursing interventions and collaboration with physicians
• Monitors patient’s response to treatment, and adjusts care as needed
• Assist with transporting patients’ to and from appointments
• Educate patients and their families in healthcare-related information
• Support patients’ physical and emotional needs
• Communicate with other members of the health care team, such as physicians, pharmacists, social workers, and technicians, regarding patients’ progress
• Assist with developing and maintaining detailed patient records, including charting, correspondence, and reports
• Use medical equipment, such as catheters, oxygen, and insulin pumps

• Monitor patients’ progress and notify physicians of the results
• Provide services for homebound patients, including medication administration, monitoring vital signs, and assisting with activities of daily living

• Report to management on patient health, and collaborate with other disciplines to ensure a smooth transition for patients and continuity of care

Requirements And Skills:

• Bachelor’s degree, RN
• Minimum 2 years’ home care experience
• Strong customer service skills
• Ability to work independently with minimum supervision
• Ability to handle multiple assignments simultaneously
